Job Description:

As a Care Manager / Scheduler at Brightwood Health, you will play a pivotal role in ensuring the seamless coordination of home health services while actively engaging in care management responsibilities. Your dual responsibilities will include:

Qualifications:
  • High School diploma or equivalent
  • Proven experience in administrative roles, preferably in healthcare or related fields
  • Strong organizational and multitasking abilities
  • Excellent communication skills, both verbal and written
  • Proficient in scheduling software and Microsoft Office / Google Suite
  • Understanding of care management principles and practices
Responsibilities and Duties:
  • Efficiently manage and coordinate the scheduling of home care services
  • Collaborate with Care Coordinators, healthcare professionals, and clients to optimize service schedules
  • Utilize scheduling software to maintain accurate and up-to-date calendars
  • Communicate effectively with clients, caregivers, and team members regarding scheduling changes
  • Provide care management support, ensuring the delivery of high-quality, person-centered care
  • Conduct assessments, develop care plans, and monitor the progress of clients
  • Collaborate with healthcare providers to ensure comprehensive and effective care delivery
  • Assist in maintaining compliance with scheduling regulations, care plans, and agency policies
  • Participate in required training and professional development activities
