As a Care Manager / Scheduler at Brightwood Health, you will play a pivotal role in ensuring the seamless coordination of home health services while actively engaging in care management responsibilities. Your dual responsibilities will include:
- High School diploma or equivalent
- Proven experience in administrative roles, preferably in healthcare or related fields
- Strong organizational and multitasking abilities
- Excellent communication skills, both verbal and written
- Proficient in scheduling software and Microsoft Office / Google Suite
- Understanding of care management principles and practices
- Efficiently manage and coordinate the scheduling of home care services
- Collaborate with Care Coordinators, healthcare professionals, and clients to optimize service schedules

- Communicate effectively with clients, caregivers, and team members regarding scheduling changes
